New Patient Information
Welcome to Dr. Chitra P. Naik’s dental office! We hope you enjoy your time here. Before you become a new patient, we recommend that you take a few moments to read through our new patient page. In it, you’ll find all the information you need to prepare for your first appointment. This includes expectations for your first visit, patient forms you’ll need to fill out and financing information. If you have any questions, don’t hesitate to contact our dental office directly!
Your First Visit
When you first arrive for your scheduled appointment, you’ll be met with a friendly greeting by one of our team members. As a private practice, we don’t want to keep you waiting on your necessary treatments. After filling out your new patient form and sitting for a few moments in our waiting room, you’ll be called in to meet Dr. Naik and relax in one of our dental chairs. After your appointment, we’ll get you scheduled for your next routine checkup and cleaning.
New Patient Forms
All new patients are asked to fill out a new patient form so we can learn as much as possible about the current state of their oral health prior to the exam. These forms also inquire about your general health, including any medications you may currently be taking. The more we know about your current health, the more effectively we can provide care that is more catered to your individual needs.
Dental Insurance Welcome
Dental insurance makes the process of getting the routine treatments you need a lot more affordable. Our dental office is proud to be in-network with most major insurance providers. We currently accept plans from the following companies:

Financing Available
Don’t have dental insurance? Need help covering the costs of another complex treatment? Our dental office works in tandem with third-party financing companies like CareCredit. Via their financing plans, you can cover the cost of a treatment in more manageable, monthly payments. In most cases, these payments carry little to zero interest attached!
